Enable Patients to Receive Medically Tailored Meals to Enhance Surgical Readiness and Post Surgery Outcomes

Mend, a life sciences company operating at the intersection of nutripharma and digital behavioral health, announced today they will be partnering with Umoja Food For Health, a company aimed at reducing nutrition insecurity, addressing social determinants of health, and the prevention or management of chronic diseases across the country. Umoja will be implementing UpgraidTM by mend, a digital health coaching platform and clinical nutrition program, which enabling patients across the country to receive medically tailored meals pre- and post-surgery to enhance surgical readiness and post-surgery outcomes.

“Many surgical patients are challenged with comorbidities and undernourishment which raises risk of poor outcomes and/or delays their ability to get surgery in a timely way,” says Eziah Syed, CEO of mend. “Tailored and targeted nutrition is incredibly important to those preparing for and recovering from surgery and we can now offer surgical readiness and preventative nutrition to patients around the country.”

The Upgraid™ by mend digital health coaching platform and clinical nutrition program extends the care delivery model beyond the hospital setting to further recovery in the home. Set up as an SMS based and AI-enabled digital care platform, Upgraid™ helps hospital systems to streamline recovery beyond the hospital walls, alleviating the burden on providers. The remote monitoring will triage critical questions and minimize patient calls to the hospital through a team of registered nurses, which frees up staff and resources to focus on the in-hospital service. Through the program, patients receive hands-on coaching, guidance, support, and health monitoring by certified professionals.

Umoja is a minority-owned health, grocery, and logistics company with a social mission, united in the drive for equity and access to quality food and healthcare for all. With a full range of interventions - from MTM, to Medically Tailored Groceries, Produce, and Nutrition Empowerment Programs - Umoja Food For Health delivers tailored nutrition + education solutions designed to address diet-related diseases and reduce nutrition insecurity.

In 2022, mend launched a line of targeted, evidence-based, nutrapharma products along with the mend digital health coaching platform and is honored to have Umoja incorporate the new Upgraid™ platform to deliver medically tailored grocery delivery programs to individuals throughout the US.
